At the Acne Center of Michigan, our comprehensive acne scar session combines multiple treatment modalities performed together, each targeting a different scar type for a result that no single treatment can achieve alone.

The comprehensive acne scar protocol

All of the following steps are performed in a single session, beginning with oral sedation and topical anesthesia starting 2 hours prior to the procedure:

Chemical peel to blend

A light chemical peel including Jessner solution and 20% TCA is applied to the entire face to blend non-scarred areas.

CROSS for ice pick scars

100% TCA CROSS into ice pick scars and/or Fotona SP Dynamis 0.5mm ablative laser into narrow ice pick scars and pits.

Laser smoothing for box car scars

Fotona SP Dynamis fine-focused laser smooths the edges of box car scars and irregular mounds to blend them into the surrounding skin.

Subcision for rolling scars

Tumescent anesthesia with Taylor Liberator subcision releases the scar bands that pull rolling scars downward.

Fractional ablative laser

Fractional ablative Erbium laser or Fractional ablative CO2 laser, selected based on the patient’s skin type, resurfaces scarred collagen.

Punch excision

Punch excision or elliptical excision surgically removes any remaining deep individual scars.

The comprehensive session addresses ice pick scars, box car scars, and rolling scars simultaneously. Each scar type receives a specific treatment modality: CROSS or ablative laser for ice picks, subcision for rolling scars, fine-focused laser for box car edges, and fractional ablative laser for overall resurfacing.

Our protocol uses conscious sedation (oral sedation plus topical anesthesia beginning 2 hours before the procedure). The patient is drowsy but awake and is monitored throughout. Most patients do not remember much afterward. Overall, the procedure is well-tolerated and comfortable.

It generally takes 7 to 10 days for scabs to resolve and the skin to re-epithelialize. Redness will persist, often for several months. No significant physical exertion is permitted for 2 weeks after the procedure. Our patients are confined to their home or hotel for a minimum of 7 days.

Yes, out-of-town patients are welcome. We prefer that you remain in the area for 1 week after the procedure. We do not recommend traveling on an airplane for 1 week post-procedure. If driving from a distance, be aware we may need you to return for a follow-up if we have concerns about your recovery. The patient will send photographs to Dr. Singer daily for the first week.
